Supported Living (ARS): Supported Living is a program designed to provide special modified living services and related training supports that are specifically planned and directed toward supporting each individual to develop the necessary daily living skills and self-care skills in order to achieve the highest possible level of independent living in the community. In short, these modified living services are intended to lend supports to the individual to become or remain self-supporting and more self-sufficient through community based programming in an environment that can offer protection for individuals who are at high risk.
Note: PAC has a majority of its residents on this program.
Waiver (DDS): DDS Alternative Community Services is a Medicaid program that has specifically designed services for persons with severe disabilities under the Medicaid Waiver program. Our agency is an Organized Health Care Delivery System Provider which allows for sub-contracting services with qualified entities and/or providing services with our own employees. These services can include: Supportive Living and Alternative Living Services; Community Services; Respite Care; Non-Medical Transportation; Adaptive Equipment; Environmental Modifications; Specialized Medical Supplies; Consultation; and Crisis Intervention. These are specialized services designed to provide the person with an alternative to institutional care and are individually contracted.
Waiver Case Management (DDS): This service involves providing case management for Waiver consumers who have chosen our organization to provide these services. Services include developing the Individualized Service Plan/Plan of Care; linkage with appropriate community resources; coordination of services; and monitoring.
Waiver Supported Employment (DDS): Some Waiver consumers qualify for special supported employment situations under the Waiver program. These services are similar to those found in the regular supported employment program.
Integrated Supports (DDS): Integrated Support Services is designed to give specific services to DD individuals living in the community in order to help them become or maintain an independent lifestyle. It can include Supportive Living, Alternative Living, Day Habilitation Services, Respite Care and Non-Medical Transportation. The assessment will determine the needs and the Individualized Service Plan will detail the planned services. Providing the necessary supports needed to be integrated into the community and promote independence is what these services are designed to do. This is a separate negotiated contract for services.
